How FHIR fits into an OpenEMR environment, and what a real integration involves.

OpenEMR includes FHIR support. Turning that into a working integration with an external application involves more than enabling an endpoint.

The moving parts

An authorization model (SMART on FHIR scopes, client registration, consent), resource mapping between OpenEMR’s data and the FHIR resources the consumer expects, terminology mapping for codes, and handling of the resources OpenEMR exposes versus those the use case needs.

Coexisting with HL7 v2

Most OpenEMR environments also have HL7 v2 interfaces for labs, ADT or billing. A common pattern is an integration layer that consumes v2 events and exposes or updates FHIR resources, so external apps get a modern API without disturbing existing interfaces.

What to plan for

Testing against real consumer apps, monitoring of the FHIR endpoints, versioning as the FHIR profile evolves, and a security review of the authorization model.
