The sequence

Assess and map
Analyze the source data: patients, clinical data, documents, appointments, insurance, billing, historical records. Define the mapping to OpenEMR’s model, including how to handle data that does not fit cleanly.
Validate before and after
Check source data quality up front. After each migration run, reconcile record counts, spot-check clinical accuracy, and verify that documents and relationships survived.
Cutover
Plan the cutover with a fallback. Keep the legacy system readable during transition. Do a post-migration verification pass with clinical and billing staff.
What not to promise
Never promise zero data loss. Promise a defined, validated, reconciled migration with known and documented exceptions.
