Assess first

Catalogue every core modification, every integration, the version gap, the infrastructure, and the workflows that depend on custom behavior. This is the Sense stage.

Re-home the customizations

Move each core modification to a module or an external service integrated through the API. The goal is a core close enough to stock to take upstream releases again.

Modernize the surroundings

Put an integration layer in front of the HL7 and billing interfaces. Move to a supported, monitored infrastructure. Address the specific performance bottlenecks — usually database and query level.

Then upgrade, then extend

With customizations re-homed, upgrade to a current version. Now new capability — automation, analytics, AI — can be added on a stable base.
